Our ability to do so now, and into the future, depends on your commitment to your oral care; therefore, payment is due at the time service is rendered unless other arrangements have been made before to your visit.

** As part of our commitment to you, we will assist you in maximizing your insurance benefits by completing and submitting your claim forms. Your estimated co-payment is due at the time of service. This is only an estimate based on your specific plan outline. Once reconciled by your insurance company, any overpayment on your part will be refunded or credited to you and any unpaid balance will be due by you once your statement is received.
